視力検査の国際比較について

今日の医療英会話では「視力」という項目があり、アメリカ・イギリスと日本の根本的な違いを説明していた。日本の視力検査は0.1 から2.0 までの指標で測定する。かつては両眼ともに2.0 だったのが、右0.3, 左0.5 くらいになっているという。それを決めるときの検査をするが、その時に使われる C という形を回したものをランドルト環という。これはほぼ純粋に視力検査だけで出てくる。
 
それに対して、他の先進国では、別の方法で視力を検査している。ウィキペディアによると以下のように説明されている。
 
アメリカやイギリスを含むヨーロッパ圏の国々などの場合は、『20/20』『6/6』のように分数で表す。分子が検査距離(20フィートもしくは6メートル)を表し、分母が『被験者がようやく弁別できる視標を、健常者(視力1.0の人)がどこまで下がって弁別できるかという距離』を表す。たとえば『20/40』の場合はこの被験者が20フィートでようやく見える視標を健常者は40フィートの距離から見える、という意味である。ただし簡単に言えば分数をそのまま小数に置き換えれば、小数視力の表記と一致する。すなわち、『20/40』は『0.5』に相当し、『6/60』は『0.1』に相当する。」
 
もちろんこの説明はわかりやすい。しかし、もちろん疑問がわくことは言うまでもない。0.5 を表現するのに、10/20でもいいし、 20/40でもいい。 30/60, 40/80 などなどでも OK である。 同じじゃないかと言われると、かなり違う。健常者の20フィートの視力なのか、それとも80フィートの視力なのかは、かなり違う。いい本をさがして読んでみよう。

19世紀から20世紀アメリカの自伝著者の統計

「自伝」というジャンルの歴史に関して、英語や日本語の学術書や論文を読み始めている。その中で、JSTOR からアメリカの自伝の統計的な視点を読んだのでメモ。
 
1800年から1950年までの期間に、出版された自伝に関して、どのくらいの数か、そしてどんな職業の人物が書いているのかという分析である。19世紀の最初の10年間は27点しか自伝は出版されていないが、20世紀の最初には500点、そして1930年代には1,000点を超えるという道筋を描く。そして非常に面白いのが、職業がはっきりと変化していることである。当初の自伝は、牧師と信仰者というグループと犯罪者というグループという二つのグループが非常に数が多かった。これらを合わせると当初は80%くらいであり、19世紀の前半には、これらのグループがやはり強いという印象がある。しかし、犯罪者の自伝の出版の割合が1840年代から急速に低下する。牧師や信仰者の割合はよりゆるやかではあるが減少してゆき、20世紀には急速に少なくなっていく。

もちろん一つの大きな流れとして知っておくと良いというか、知っておくべきだろう。しかし、私が見ている東京の精神病院の患者は、犯罪者と信仰者、場合によっては犯罪者と教祖の両者をそなえ、両者を自伝に書き込む患者もいる。

黒死病の墓場の混沌と人骨の秩序

感染症をもう一つ。こちらはスミソニアン博物館からのメールで、中世のイングランド黒死病に関しての記事。 国際的な考古学の雑誌である Antiquity に掲載された論文である。

人骨の考古学と病理学を組み合わせるのは、比較的新しい方法で、医学史研究者の記憶の中では、14世紀の黒死病がペストであるという方向を決定的に論証した方法であった。この論文は、もちろん考古病理学の方法を使って、年代を決定し、ペストの病跡を確定している。面白いのは、これまで都市部での埋葬ばかりであったが、農村でも考古病理学を用いた研究ができることを示していることである。それに加えて、人骨の配列や並べ方などを解釈する非常に面白い視点を使っている。埋葬されている人骨が非常に多数であり、数十点であることは、もちろん大打撃だったことを示している。使っている英語は catastrophe である。それと同時に、その多数の人骨がかなりきちんと並べられている様子は、埋葬が秩序を維持していることを意味する。ヨーロッパに黒死病が襲い掛かり、都市も農村も非常に大規模の死者が出て、葬儀が行われ、数多くの人骨が埋葬されていく。その中に秩序があるというのは、ヨーロッパの文化の一つの明るい部分であると思う。日本では人骨の扱いがやはり違う。ことに、白骨死体というと、野に放置され、風雨や陽光にさらされた「死」と強くつながっている。それとは違う形の「人骨」のイメージが黒死病に存在した。

コロナウィルスの流行と民主主義と鄧小平との関係について

 
コロナウィルスの流行 (Coronavirus disease 2019, covid 19) が注目を集めている。医学史家としては感染症の流行を深い部分で考えるヒントがたくさん現れている。ことに、今日のエコノミストの記事が非常に面白かった。
 
主題は感染症の流行と。流行地域の民主主義の関係である。データとしては、ルーヴァンのカトリック大学が公開するデータベースを使い、1960年のチベット天然痘の流行から2019年の covid-19 までを見る。横軸が人口一人当たりのGDP でその地域の富裕さをあらわし、縦軸がそれぞれの感染症の流行での死亡率を示している。それに加えて、流行があった地域が民主主義か非民主主義かという規準で、ドットの色を変えて、薄紫か水色かであらわしている。そうすると、ある意味で当然のことであるが、所得が上昇すると感染症の流行による死亡率が低下することが分かる。もう一つが、民主主義か非民主主義かによっても明らかな違いが出ていることがわかる。死亡率を較べると、現在の中国の所得でいうと、民主主義国家では100万の人口につき4人、非民主主義国家では6人となる。

非常に複雑な議論と、非常に単純化したモデルの双方が必要な議論である。そこに民主主義の概念を、適度な強調をもって入れることができる。中国の体制に対して批判的なコメントを出すと、鄧小平の有名なセリフがある。英語で書くと “It doesn’t matter whether the cat is black or white, as long as it catches mice.” これは、資本主義が発展して豊かになれば感染症の死亡率は下がるという経済的な視点である。しかし、そこには民主主義も必要であり、政治的な要素も確実に存在している。
